2. date and time you would like to meet, along with length of engagement

3. the city you would like to meet in

4. names, websites, and e-mail addresses of at least two independent providers you have met in the last six months

OR

include full legal name, business e-mail address, business website, and business phone number. For fly-me-to-you dates, I require both methods.
